Fix unit tests

Images can be uploaded with alt text

This means associating images to notes via a pivot table, we also finish
off the migration of medai from S3.

Allow files to be added to notes

This is specifically when a Micropub client is using the JSON syntax and
initially uploads the photo via the media endpoint
